Thank you for having your pet spayed and neutered!
You must download all five of the following forms. Three need to be read and signed and you'll need to return the signed originals to us with payment, and two are for your records. If you have trouble opening the forms, trying saving them to your computer and then opening them.
We work with a clinic in Madison called Shelter From The Storm Spay Me. You will drop your pet off at the Green Lake Area Animal Shelter at 5:30pm on the Tuesday before the surgery day, and you pick your pet up at GLAAS at 7:30pm that Wednesday.
Our surgery day is always the first and third Wednesdays of the month. The deadline to register your pet is always the Friday before the surgery day.
